Adult Foster Care
These licensed homes provide long-term care to individuals 18 and older with moderate to severe mental disabilities. Individuals who reside in these homes do not require 24 hour continuous nursing care, are both non-ambulatory and ambulatory and may also have secondary health disorders such as seizures disorders, coronary disease, or may have a history of aggressive behaviors, which include self-abuse, physical abuse, property destruction and elopement. Our homes are funded through federal dollars and through an individual’s Social Security.
Licensed Facilities Specialty Certification Adult Foster Care
These licensed homes provide care to individuals with developmental, physical and mental disabilities. Individuals living in this setting do not require continuous nursing services and are either developmentally disabled or mentally ill. The services within this setting are more intensive in the areas a Daily Living Skills, high behavioral needs, high medical needs and physical rehabilitation. The homes are funded through Medicaid, state and local counties and oversight is provided by the local community mental health agencies. |

In Home Services

Child Waiver

Services are provided in the child’s home. Funds are secured through the state and local Community Mental Health Agencies. The hours of support are decided in coordination with the Person Centered Plan, and can vary from two hours to 48 hours per week. Respite hours are also available to assist the family, when the parents need to leave the home and the child is unable to go with them. The services include a comprehensive plan to interact with the child in a consistent manner and to build skills and redirection, when engaged in inappropriate behaviors. This service is targeted for children who have autism and challenging behaviors.

Support Coordination is a service for developmentally disabled individuals and their family/guardian that have selected Spectrum Community Services as their service provider. With the assistance of a Support Coordinator, the client and family members work through a Person Centered Plan. This plan identifies the services, goals, objectives and outcomes for the individual during the coming year. The Support Coordinator links, coordinates, assists, and assesses what services the individual may need in order to achieve their goals. A formal individual plan of service is written, and a copy is given to all parties.
The Coordinator will also assist the person in obtaining and maintaining benefit coverage such as Medicaid, Medicare, Social Security, Home Help and Bridge Cards.

Community Living Supports
|
Community Living Supports has staff available to provide direct care within the individual’s home or apartment. The service is designed to accommodate individuals who have developmental disabilities or mental illness in a setting that is their own. These services are provided on an hourly basis set by the person centered plan.
The individual typically is able to function on a higher level of independence, but needs supervision of services, as they are being trained to learn skills to live on their own with as limited support or assistance.

Supported Independent Living
|
The focus of services is on increasing the person’s daily living skills, social skills, vocational skills, community integrations and money/financial literacy skills. This program has staff available to provide direct care within an individual’s own home. Services are generally provided 24/7 in a home that is shared with roommates. This program is budgeted on a daily per diem rate.

Self Determined Services
|
Self Determined Services is a staffing service provided in consumers’ homes. Our staff will assist consumers to focus and learn how to develop normal life patterns. Consumers in this program have control of their finances and can make decisions.
This is not a 24/7 service as the individual typically has the ability to be alone within his or her home. Treatment may include self-care skills, communication, social skills, cognitive, daily living skills (housekeeping, cooking, survival skills), money management, work/employment skills and family/friend recreation activities.

Individualized clinical support and planning is provided to consumers in Kent County. Nursing, behavior and group therapy, speech, occupational and physical therapies are just a few of the ancillary services provided.
Support coordinators assist individuals through the use of a "Person Centered Plan." This unique tool prioritizes and customizes the array of services to best meet each individual's specific needs. The Person Centered Planning process addresses any cultural, spiritual, or other identified needs.
